If your child was followed by another pediatrician before coming to our practice, it is important to get your child’s prior medical records in order for us to provide the best possible care. Please complete a Medical Records Request.
We are moving and will have to leave SEBPMG. How do I get a copy of my child’s records for the new pediatrician?
If you are leaving our practice and would like us to send a copy of your child’s medical records to your new pediatrician, please complete a Records Transfer Request.
Our insurance has changed. How do we get the new information to your office?

Can I bring my child to the Drop-in Office Hours for a follow-up appointment?
No, Drop-in hours are for urgent issues only (e.g. fever, vomiting, ear ache, sore throat, etc. We are not able to handle well-checks or follow-ups during Drop-in hours.
Can anyone come to Drop-In Office Hours?
No, we offer these special hours for established patients only. New patients will need to make an appointment before they can be seen.
