New patients are always welcome! Our goal is to provide you with the best medical care for your infant, child, or teenager on an ongoing basis, whether it be for routine, ill, or urgent care. We provide care for all children from newborns through teens to age 21. Routine physical exams are scheduled 1-2 weeks in advance depending on availability. For sick or urgent visits, we make every effort to see you the same day you call for an appointment.
Please check with your insurance plan to make sure that the physician you have chosen at South East Bay Pediatrics is part of your insurance network. If you have trouble finding out whether or not we are a part of your plan, please feel free to contact us directly with your insurance information and we may be able to help you.
Forms
New Patients are required to fill out the following forms and bring them at your first appointment:
- All patients: Health History Questionnaire – general health questionnaire for all new patients
- For patients 0-11 years: Pediatric Registration 0-11 years
- For patients 12-17 years: Pediatric Registration 12-17 years
- For patients 18 and older: Adult Registration 18-21 years
These forms need to be filled out before you can be seen. Please bring the completed forms with you at the time of your appointment. As a reminder, please arrive early in order to verify your insurance coverage prior to your appointment.
